ILLiad User Survey: Informed Consent Section

INTRODUCTION

Participants are invited to participate in an informal assessment of current and future library services. The purpose of the survey is to gather input from University of Tennessee users of interlibrary and document delivery services. Input will inform UT Libraries faculty and staff of possible changes and enhancements for existing and future services.

INFORMATION ABOUT PARTICIPANTS’ INVOLVEMENT IN THE STUDY

Participants will complete an anonymous five question, free-text web form soliciting feedback on library services. The questionnaire will take from 2-5 minutes to complete, depending on respondents’ thoroughness in composing. The questionnaire will be available for anyone to complete until January 2008.

RISKS

There are no risks with this survey. Questionnaire is an anonymous web-based survey. No personal data will be collected. Any self identifying information respondents provide will be removed or otherwise edited before general results are shared with UT Libraries faculty and staff involved in interlibrary and document delivery service.

BENEFITS

UT Libraries will gain valuable, direct, and honest critiques of current and possible future services from the people who use our services. UT Libraries will incorporate user feedback into projects slated to improve our interlibrary and document delivery services.

CONFIDENTIALITY

This is an anonymous survey. No personal data is required or will be captured. Any self-identifying questionnaire responses will be removed or otherwise edited. Results captured will remain secure in a password protected library network, accessible only to the principle investigator and appropriate library information technology managers. The information in the study records will be kept confidential.

Final results including both paraphrased and verbatim answers may be published in internal library reports and external publications and presentations. No reference will be made in oral or written reports which could link individual participants to the study.

PARTICIPATION

Your participation in this study is voluntary. You may decline to participate without penalty. If you decide to participate, you may withdraw from the study at anytime without penalty and without loss of benefits to which you are otherwise entitled. If you withdraw from the study before data collection is completed your data will be returned to you or destroyed.
